Perhaps, for the future, one of two things could be done.
1) Advance notice of a couple weeks to operators so we can be aware to be on the lookout for certain titles or remove them to personally test before reactivating them on the stations.
2) When a title get switched, have it automatically deactivate on all stations with a notice to have the operator check, re accept and now with the knowledge that there may be issues so they arent caught with their pants down.
However, i do agree that these are things that should be part of the QA checklist over there for when making the switch to avoid problems.
