Would anyone be willing to share their disclaimer/waiver of liability for us to use for our arcade? Has anyone had anyone have any health issues/seizures during use?

We did not had any issues since we open. Best thing is to ask customer if they have vertigo or epilepsy since this conditions are more likely to create problem in VR.
Used google forms for ours. It is free and included the html text to put it on your webpage even
